How it goes

  1. Upload your medical documentation.

A printout, a photo of a page, a discharge notice, an email from the hospital. Upload what you have.

  2. Check the details.

goyo shows the patient, the place and the dates. Fix anything wrong and pick what you need.

You get one letter for each office that can help. You read each one, change what you like and send it. When you mark a letter sent, goyo shows the date the office should answer.

What to gather

Start with what you have, since you do not need everything.

  • Any notice you were handed, such as a discharge notice. If a notice has a deadline, act on the notice first.
  • Discharge papers, test results and notes from the portal or the hospital.
  • Messages and emails with the hospital, kept in writing when you can.
  • Your own notes: the date and time of each call or talk, who was there, and what was said.
Keeping your papers in good order

Keep each original exactly as you got it. Put your highlights and notes on a separate copy. Save a new copy when something changes, such as a new note, a correction or a discharge plan. Keep the older copy too.

After a call, send a short message: "Thank you for speaking with me today. My understanding is [summary]. Please tell me if I have it wrong." Keep the reply.

If papers are missing, get a copy of your full record.

Four things you can do, and how long you have

You can do more than one, and your notice has the deadline, so start there.

Being sent home too soon

Appeal by the day you leave (42 CFR 405.1205).In a hospital, call the reviewer named on your notice before you go. From rehab, a nursing facility or home health, call as soon as you get the notice, and no later than noon the day before the last covered day. Appeal a discharge, fast

Care was unsafe or wrong

Put a complaint in writing. The hospital sets its own days and must give you a written answer. You may tell more than one office. Each office has its own rules. Privacy complaints to the federal government are due within 180 days (45 CFR 160.306). Report a care or safety problem

A record entry is wrong

The office has 60 days to answer (45 CFR 164.526).They may take one more 30 days, but only if they tell you in writing. Correct the record

You need a copy of your record

The office has 30 days to give it to you (45 CFR 164.524).They may take one more 30 days, but only if they tell you in writing. In a nursing home it is much faster (42 CFR 483.10(g)(2)). Get a copy of your full record

Before anyone leaves, read Questions to ask before you leave. Your state may have its own rules and deadlines, so check the notice you hold.

Who decides

You decide what to send, and a doctor or lawyer can advise you on care or your rights, while goyo helps you read and write.

Check it against the original

A page can be read wrongly, and some pages cannot be read at all, such as a blurry photo. Before you rely on a date, a dose or a name, look at the page itself. Something missing from a page is not proof that it did not happen.

Your privacy

goyo reads the papers you add so it can write your letters. To do that it may send the pages a job needs to an outside AI service. It keeps the papers, the text it reads and the letters in your account until you delete them. Some copies may stay a short time in backups.

You can delete your records and your account yourself, from your account page. Deleting here does not delete copies that a hospital, an agency or someone you shared them with already holds.

Send records only through goyo, not by email or the contact form.
