About you and the patient
Pick a letter
Ask whether the hospital logged what happened as a safety incident
Send it to the hospital's risk manager or patient safety officer, with a copy to patient relations.
Ask the hospital to save your record and every change made to it
Send it to risk management and the medical records office, with a copy to the privacy officer.
Ask for more days before discharge
Send it to the attending doctor and to case management. If you were admitted as an inpatient with Medicare or a Medicare Advantage plan, you also have a fast appeal. You ask the Medicare review office (QIO) for it before you leave, by the planned discharge day.
Ask for results to be reviewed with you before discharge
Send it to the doctor in charge, and to the covering doctor if yours is away.
Ask a facility to follow the outside doctor's orders
Send it to the attending doctor, with a copy to the director of nursing.
Ask why a medicine stopped between visits
Send it to the office of the doctor who prescribed it. A phone call to that office is faster than a letter.
Ask for a second opinion or a transfer
Send it to the attending doctor and to case management.
Put in writing that care changed after you raised a concern
Send it to the administrator, with a copy to patient relations.
